Responsibilities: Participate in development and implementation of service plans and initial assessments for nursing clients. Regularly assess clients’ needs, complete service plans, and document observations at each visit. Perform ambulation, transfers, positioning, wound care, catheter care, ventilator care, IV therapy, lab draws, injections, diabetic teaching, colostomy teaching, tracheostomy care, parenteral feedings teaching, post‑op recovery, medication administration teaching, and monitoring. Coordinate with physicians, PT, OT, and other care team members. Educate patients and families on home health procedures and strategies. Recommend tools or devices that improve quality of life. Conduct regular evaluations and reevaluations of nursing needs (minimum every 60 days). Take verbal orders and make referrals to other disciplines as indicated. Conduct training (Medication Administration, Orientation, In‑service). Supervise CNAs, PCAs, and nurse performance; manage records. Investigate incidents, grievances, and accidents. Communicate changes in visit assignment and scheduling to agency staff. Maintain training requirements per governing authorities. Qualifications: 2+ years’ nursing work experience. Current RN licensure in state of practice; meet recurring training requirements. Current CPR certification (American Heart Association). Basic health and tuberculosis screening. Ability to lift up to 50 lbs. Independent and collaborative work style; strong communication in fast‑paced environment. Valid driver’s license, vehicle insurance, and reliable transportation.
