Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs and physician direction
• Collaborate with an interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program development, and coordination of continuing services
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines
• Effectively manage individual, concurrent, and group treatment sessions based on patient care needs while demonstrating flexibility in caseload management
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific state practice acts and all regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in good standing in the state of Indiana (or eligibility to obtain)
• Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
• Ability to work Monday–Friday with weekend rotation availability as needed

Preferred Experience / Skills

• Experience in an inpatient rehabilitation or acute care setting
• Familiarity with interdisciplinary team collaboration and discharge planning processes
• Experience supervising or mentoring Physical Therapy students
• Active involvement in professional development, continuing education, or professional organizations
