Age Specific Responsibility
Must be able todemonstratethe knowledge and skills necessary to provide careappropriate tothe age of the patients served in assigned department. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate informationneeded to identify each patient's requirements relative to age-specific needs and to provide the care needed as described in the department's policies and procedures.
Summary
Assumes full responsibility for evaluating patients' need for occupational therapy.Plans and executes individual treatment programsin accordance withthe state of Michigan licensure and OT scope of practice.Keeps treatment skills updated through continuing education,evidence basedresearch, and program development. Perform job responsibilitiesin accordance withthe program's strategicobjectivesand goals to carry out organizational strategicobjectives.

Staff will provide patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.
Provides patient care. (Q)
Evaluates patient condition.
Establishes treatment goals in collaboration withpatientand family asappropriate, and in collaboration with the treatment team. Treatment goals should be interdisciplinary and patient-centered whenever possible. Establisheswrittentreatmentplan, andadministers treatment for each patient according to the stated goals.Delegates responsibilities to occupational therapy assistants asappropriate.
Re-evaluatespatient'sstatus and effects of treatment.Revises treatment plan as necessaryReviewsrevised goals with patient and family asappropriate.
Evaluatespatient'sneed for medical equipment and orders theappropriate equipmentas necessary.Evaluates equipment upon delivery and instructspatient,patient'sfamily, and health care workers on its use.
Participates in planning for patient discharge by considering and communicating with patient, family, community support/services, and environmental limitations.
Demonstrates competence in relevant software and hardwarerequiredto meet job responsibilities. (Q)
Documents patient's status and progress throughinitialevaluation, progress notes, and discharge summary according toestablishedinternal/external time standards and adherence to reimbursement guidelines. (Q)
Communicates with others to integrate occupational therapy treatment with all aspects of patient care. (Q,S)
Participates in conferences/discussions as needed and exchange of written records with other interdisciplinary team members to integrateoccupationaltherapy treatment plan.
Instructs the patient and the family in occupational therapy procedures as part of providing patient and familyeducationincluding writteninstructionsas necessary.
Attends various meetings and in-services as designated. (Q,P)
Follows all hospital policy and procedures including but not limited to patient confidentiality and infection control. (Q)

Essential Job Qualifications
Graduate of an American Occupational Therapy AssociationaccreditedOccupational Therapy program.
Certification by the National Board for Certification in Occupational Therapy.
Licensure with the state of Michigan.
Possesses and maintains BLS certification to meet department/program protocol.
