Tennessee Licensed Care Coordinator

To support a growing practice, the full-time Tennessee Licensed Care Coordinator will provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible members, building trusted relationships and addressing both medical and social needs in a remote setting. Key Responsibilities Conduct outreach to educate members about program benefits and assess their status to develop personalized care plans Manage a caseload of members, facilitating coordination of care with providers and ensuring timely communication of medical information Document actions taken in a care management platform and identify needs related to Social Determinants of Health Required Qualifications Active and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license Fluency in both Spanish and English 2+ years of experience in care coordination, case management, or related fields 1+ years of remote work experience Experience working with Medicaid and/or Dual Eligible patients
