Summary The LPN is responsible for observing, recording, and reporting changes in the patient's condition—both through in‑person care and virtual healthcare modalities—and for assisting in the development and revision of the patient‑driven holistic care plan. The LPN is a member of the patient healthcare Teamlet. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. United States citizenship; non‑citizens only eligible if qualified citizens cannot be recruited per VA policy. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time of completion. Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, Commonwealth (e.g., Puerto Rico), or District of Columbia. License history: An LPN/LVN who has had a license revoked, suspended, denied, restricted, limited, or placed in probationary status may be appointed only per provisions in chapter 3, section B, paragraph 16; qualifying under the Grandfathering Provision is available to current VHA employees who meet criteria. Grade determination: GS‑3 – basic requirements only; LPNs perform duties within scope of license. Grade determination: GS‑4 – may be appointed or advanced if meet criteria: (a) six months of qualifying experience as an LPN/LVN; (b) graduation from an approved school and one year of experience in nursing care; or (c) graduation from an approved school of at least 24 months. GS‑4 LPNs perform GS‑3 duties but exercise greater judgment and require less supervision. Grade determination: GS‑5 – requires at least one year of qualifying experience at GS‑4 or equivalent; demonstrates ability to provide full range of practical nursing care; works with RN/MD to orient/train less experienced staff; provides patient education; serves as a responsible member of nursing team. Grade determination: GS‑6 – requires at least one year of qualifying experience at GS‑5 or equivalent; competent in complex patient care, medication administration, emergency interventions; demonstrates judgment in selecting procedures; prepares and administers medications; fosters respectful relationships with patients/families; supports diagnostic test procedures; serves as preceptor for complex tasks. Duties VA Careers - Licensed Practical Nurse: Total Rewards of an Allied Health Professional. Major duties include: makes observations and thoroughly documents care and the patient's response. Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher‑level intervention. The LPN performs, reports, and documents assigned nursing care in the Primary Care Clinics for both in‑person and virtual modalities—including oxygen administration if applicable—and adheres to the PACT Nursing Protocols within scope of practice. Medication administration, including immunizations. Clinical reminders with appropriate consults and alerts as indicated. Check expiration dates of medications and supplies. Delegated procedures and treatments: assists with minor procedures—obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Promotes whole health initiatives—including Tele‑MOVE, guided imagery, and conducts chart audits as assigned. Participates in performance improvement initiatives. Facilitates Clinical Video Technology appointments. Checks in and checks out Veterans daily. Collaborates with the PACT team to review future appointments. Participates in daily huddles. Secure messaging within scope; responds with recommendations; performs other duties as assigned by supervisor (Assistant Nurse Manager or Nurse Manager). Adheres to standards of practice and safe techniques in preparation and administration of medication. Demonstrates understanding of age‑and population‑specific characteristics affecting the Veteran’s health care needs. Promptly recognizes urgent or emergent patient care situations, seeks assistance, and intervenes as directed. All other duties as assigned. Work schedule: 7 a.m.–7:30 p.m. and 7 p.m.–7:30 a.m. Pay: Competitive salary with regular increases; higher step rates may apply for higher qualifications. Paid time off: 37–50 days annual paid time off; 13–26 days annual leave, 13 days sick leave, 11 paid federal holidays per year. Selected applicants may qualify for credit toward annual leave accrual based on prior work experience or military service. Parental leave: after 12 months of employment—up to 12 weeks paid parental leave for birth, adoption, or foster care placement.
