Your next nursing opportunity starts here. We are seeking a skilled and compassionate Registered Nurse (RN) with strong OASIS expertise for a full-time, 13-week home health assignment. This role is perfect for professionals who excel at clinical documentation, independent patient care, and collaboration within a multidisciplinary team.

Schedule:

• Full-time, Monday through Friday
• On-call responsibilities likely

Key Responsibilities:

• Deliver in-home skilled nursing care to patients with varying needs
• Complete thorough and accurate patient assessments, including timely OASIS documentation
• Collaborate with physicians and care coordinators to ensure personalized care plans
• Administer wound care, medication management, patient and family education, and ongoing disease monitoring
• Communicate patient status updates and changes in condition promptly to the care team

Desired Qualifications:

• Current, active RN license
• Minimum of one year of recent home health experience as an RN
• Proven proficiency with OASIS documentation (required)
• Experience with HomeCare HomeBase (highly preferred)
• Strong organizational and time management skills to handle structured daily visits
• Reliable transportation and valid drivers license
