Work Environment\nThis position is fully remote, allowing you to work from the comfort of your home while needing to occasionally visit our hospice center in Chandler, Arizona. Key Responsibilities\n- \n
- Conduct comprehensive evaluations and assessments to determine the needs of children and families. \n
- Actively participate in the development and implementation of Individualized Family Service Plans (IFSP). \n
- Monitor and evaluate outcomes as a member of the multidisciplinary team. \n
- Provide continual support and guidance to families to promote their child's development and well-being. \n
- Ensure compliance with all Federal and State regulations and standards for service delivery. \n
- Utilize evidence-based practices to develop personalized strategic plans in collaboration with families. \n
- Engage with children and families to build rapport, facilitating a conducive learning environment. \n
- Implement natural environment teaching methods as part of your interaction with families. \n
- Make necessary recommendations to maximize efficacy in service delivery based on child development progress. \n
- Stay informed about the latest practices in early intervention and family support. \n
The primary focus of this role is on young children, particularly those with disabilities or developmental delays, and their families. You will assist in helping infants and toddlers develop essential skills in the areas of physical, cognitive, social/emotional, and communication. This position is centered on creating a nurturing environment where these children can thrive, which is key to their future success.
Qualifications\n- \n
- Master's Degree or Doctorate in Social Work. \n
- Must be licensed by the Department of Public Health, with an LCSW designation required. \n
- A valid driver's license is necessary for potential travel. \n
- Ability to adapt approaches to match diverse learning styles and individual needs. \n
- Excellent communication and organizational skills are essential for effective collaboration. \n
- A respectful attitude towards others, with a strong ability to work as part of a team. \n

- A minimum of one year of experience in providing services within an early intervention context. \n
