Shift: 08:00 - 16:30 Days: M-F Weekend rotation may be included.

Valid nursing license Relevant certifications as required Experience in post-hospital or long-term care settings

Provide direct patient care and support for rehabilitation needs. Monitor patient progress and report changes to the healthcare team. Assist with daily living activities and ensure patient comfort. Collaborate with multidisciplinary teams to plan and implement care. Maintain accurate patient records and documentation. Comply with all healthcare protocols and safety regulations.
