Base Pay and Degree Base Pay $50.00 / Hour Required Degree 2 Year Degree Contact Information Phone View phone number on click.appcast.io Description Position Summary: Provides skilled nursing care to patients in hospitals, rehabilitation facilities, hospice, and/or home health settings. Coordinate care with the patient, family, and referring agency, and assume responsibility for the coordination of care. Provides service in accordance with the agency philosophy, standards, and policies. Essential Functions Responds to a request for initial assessment within 24 hours. Develops a written plan of care for each patient, assessing the physical and psychological needs of the patient and documenting the findings. Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educate the patient and family regarding pain management. Performs patient care responsibilities, considering needs specific to the standard of care for the patient’s age. Demonstrates knowledge of medications and their correct administration based on the age of the patient and the patient’s clinical position. Provides skilled nursing care as needed. Obtains all necessary physician orders and collaborates with the physician as the patient's needs dictate. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc. Follows up with obtaining lab results and notifying the physician of any significant results. Re-evaluates the need for continued care on an ongoing basis; initiates recertification as necessary. Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient/family cultural, ethnic, and/or personal needs or preferences. Supervises Home Health Aides on an as-needed basis. Initiates needed referrals to other disciplines and/or community resources. Obtains needed supplies and equipment to provide care. Knowledgeable of current federal, state, and agency regulations concerning patient care. Adheres to all Agency, payroll, billing, and documentation policies and procedures. #J-18808-Ljbffr
