In this pivotal role, you will coordinate hospice services to ensure the provision of optimal care through timely and accurate communication and documentation. The successful candidate will embody Bloom's standards of quality and compassion, continually ensuring alignment with our commitment to superior patient care.ResponsibilitiesAssessment and Eligibility: Reviewing and assessing patient medical records and information to determine eligibility for hospice care. Communicating with healthcare providers, physicians, and other involved parties to gather necessary information.Communication: Interacting with patients, their families, and healthcare professionals to explain hospice services and answer any questions. Providing emotional support and guidance to families during the difficult decision-making process of choosing hospice care.Documentation: Ensuring accurate and thorough documentation of patient information, medical history, and other relevant details. Managing the intake paperwork and obtaining necessary consents for hospice care.Coordination: Collaborating with other members of the hospice team, including nurses, social workers, and chaplains, to coordinate the admission process.
