CenterWell Home Health seeks a compassionate Occupational Therapist to provide in-home evaluations and therapeutic interventions for adult and geriatric patients. You will assess functional abilities, develop and implement individualized care plans, teach adaptive techniques, recommend DME and home modifications, and collaborate closely with nurses, PTs, physicians, and caregivers. This role emphasizes patient autonomy, safety, and measurable outcomes while documenting care in EMR.
