Position Details Co-Occurring Therapist – Assertive Community Treatment (ACT) team Location: 912 Main Street, Stroudsburg, PA 18360 Rate: $26.94 an hour + mileage reimbursement Responsibilities Collaborate with psychiatrists, primary care providers, and ACT team members to support integrated treatment planning for individuals with co-occurring mental health and substance use disorders, including coordination of medical, psychiatric, and community-based care. Provide ongoing counseling, education, and clinical support to individuals and families, including psychoeducation on substance use disorders, participation in treatment planning, and assistance with linking to community resources and services. Deliver community-based ACT services, including home visits and field-based support, while contributing to clinical documentation, staff meetings, and, when needed, court‑related reports or testimony. Learn more about our full benefits package - Qualifications Master's degree in human services or related field. 2 to 3 years of experience working with individuals with co-occurring substance abuse/mental health disorders (required).
