Hospice Admissions RN - Registered Nurse

Job Title: Registered Nurse – RN, Job Location: Knoxville, TN, Job Shift: Tuesday, Thursday, Friday 12‑hour shifts Position Overview The RN provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Responsibilities Coordinate the total plan of care with other healthcare professionals involved in the patient’s care, maintaining continuity by planning and exchanging information with the physician, agency personnel, patient, family, and community resources. Make the initial and/or comprehensive nursing evaluation visit, determine the primary focus of care, develop the plan of care within state‑specific guidelines, and ensure patients meet home‑health eligibility and medical‑necessity guidelines as defined by the payer. Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and response to home health care. Provide high‑quality clinical services within the scope of practice defined by state laws governing nursing practice, in accordance with the plan of care, from admission through discharge. Must Haves One (1) year of clinical RN experience. Current RN license in the state where services are rendered. Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Ability and willingness to reliably commute to multiple patient residences daily within the geographic area assigned.
