Case Manager Registered Nurse

As a not-for-profit, full-service, 318-bed community hospital, Sibley offers medical, surgical, obstetric, oncology and orthopaedic treatment, intensive care, skilled nursing inpatient services and a state-of-the-art 24-hour emergency department. Sibley's campus is also home to Johns Hopkins Kimmel Cancer Center and The Johns Hopkins National Proton Therapy Center.

What you will do:

In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family and care team. Develops a sustainable, safe transition plan appropriate to the patients' needs and resources in collaboration with the patient/family and interdisciplinary care team.

Qualifications:

• Completion of an accredited BSN Nursing Program (RN) required.
• Minimum 2 years of experience in clinical nursing required; minimum of one-year in case management, and/or discharge planning preferred.
• Accredited Case Manager i.e. - Registered Nurse (ACM - RN) / Certified Case Manager (CCM) preferred
• Licensed to practice as a Registered Nurse in the District of Columbia.
• CPR- Basic CPR required

Salary Range: Minimum $43.25/hour - Maximum $67.04/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
