Home Health Registered Nurse

Job Summary

Seeking a Registered Nurse to provide comprehensive home health care, coordinate patient services, develop and manage plans of care, and educate patients and caregivers while maintaining compliance with regulatory standards.

Responsibilities:

• Provide nursing care through the nursing process and maintain professional nursing standards.
• Supervise CNAs and coordinate care with other healthcare professionals.
• Complete comprehensive initial and ongoing patient assessments.
• Assess physical, psychosocial, environmental, and community needs.
• Initiate, evaluate, and modify Plans of Care as needed.
• Establish discharge plans with patients, physicians, caregivers, and other disciplines.
• Complete OASIS assessments and required documentation.
• Provide patient care in accordance with Federal, State, and agency regulations.
• Assess patient and caregiver education needs and provide appropriate teaching.
• Maintain accurate documentation according to regulatory requirements.
• Promote a clean and safe patient care environment.
• Maintain patient privacy and confidentiality.

Requirements:

• Home Health experience required.
• Active BLS or willingness to obtain required.
• Valid driver's license and auto insurance required.
• Active Illinois nursing license required.
• Strong Med/Surg experience with some Home Health experience may be considered.
• OASIS and Start-of-Care experience strongly preferred.
• COVID and Flu vaccination required; declinations will not be approved.
• Local candidates preferred based on territory coverage.
