Per Diem / PRN Nurse RN - Home Health Admission - $40-50 per hour

Job Title and Location Admissions RN – Home Health, Denton, Texas. Primary Functions Perform admissions and administer nursing care for patients of all ages in their place of residence. Coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume responsibility for coordination of care. Responsibilities Works in patients' homes in various conditions and locations. Assess home‑health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs. Ability to work a flexible schedule; ability to travel locally and respond to PRN emergency calls. Develop and document the plan of care in correlation with the assessment. Provide nursing care utilizing infection control measures (OSHA). Work with the interdisciplinary group to ensure patient and family needs are met. Communicate with attending physicians, home‑health physicians, and other physicians involved in the patient’s care. Submit all required paperwork in a timely manner. Demonstrate commitment, professional growth, and competency. Meet productivity standards. Perform on‑call responsibilities and provide on‑call service to patients and families as assigned. Provide effective communication to patients/families, team members, and other healthcare professionals. Qualifications Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing. Licensure: Current state license as a Registered Nurse; proof of current CPR certification; current driver’s license. Experience: Two years as a Registered Nurse in a clinical setting; one year full‑time home‑health experience preferred but not required. Knowledge of OASIS highly preferred but not required. Skills: Strong assessment and clinical skills; excellent communication skills; ability to work independently and as part of a team; familiarity with infection control measures (OSHA); knowledge of state and federal regulations; good interpersonal skills. Home Care Home Base (HCHB) knowledge highly preferred. Transportation: Reliable transportation, valid driver’s license, and current auto‑liability insurance.
