RN Home Health-{170030} Location: South Portland, ME 04106 · Field coverage in Sanford/Springvale area extending to Kennebunk On-call: May be required if requested (based on branch/staffing needs) Productivity expectation: ~30 units/week Float required: Yes (local travel) Hourly Pay: $55.00/hr. Hours: 40 hrs./week Overview We are seeking an RN Home Health for a contract opportunity supporting home health patients in the greater Sanford/Springvale, ME area (extending to Kennebunk ). This RN will provide skilled nursing care in the home, coordinate with the interdisciplinary team, and ensure accurate clinical documentation and continuity of care. Key Responsibilities Provide skilled nursing care to patients in their homes across varied conditions/settings Assess patients and identify physical/psychosocial/environmental needs Develop, implement, document, and evaluate the Plan of Care (POC) Maintain timely and accurate documentation in the clinical record Educate patients/families on medications, diet, home safety, and self-care practices Apply infection control practices consistent with OSHA standards Supervise/guide home health aides (HHA/STNA/LVN) as appropriate Perform pain assessment and implement pain management techniques Coordinate referrals & communicate with care team to support continuity of care Support compliance needs for third-party payers Required Qualifications Active RN license (state-appropriate). BSN preferred CPR/BLS certification (current) Homecare Homebase (HCHB) & OASIS experience-required 2+ years home health experience-required 1-2 years RN experience in a clinical care setting-required Valid driver’s license + reliable transportation/insurance Must be comfortable with skills such as venipuncture/IV care and routine med-surg nursing in the home Preferred CHPN certification preferred Keywords RN, Home Health, Visiting Nurse, OASIS, HCHB, Homecare Homebase, Field Nurse, Med-Surg, IV Care, Wound Care, Care Coordination
