Physician Assistant
Black Women's Mental Health Institute
Role: Physician Assistant responsible for conducting in-home and virtual clinical evaluations for vulnerable patient populations. Key Responsibilities Perform comprehensive patient histories and physical examinations. Manage chronic conditions and reconcile medications. Develop and implement evidence-based care plans aligned with value-based care objectives. Coordinate follow-up care in collaboration with interdisciplinary teams. Utilize data and analytics to inform clinical decisions. Document all patient interactions accurately in electronic health record (EHR) systems. Qualifications & Skills Clinical experience in patient evaluation and management. Strong understanding of chronic disease management and medication reconciliation. Ability to develop evidence-based care plans. Excellent communication and teamwork skills. Proficiency with EHR systems and data utilization.
