divh2Registered Nurse (RN)/h2pThe Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of patients. The staff RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice and coordination of care with other health team members./ppAs a Case Manager RN you need to know how to:/pulliCoordinate the care and services for patients identified as needing assistance or meeting Care Management criteria./liliCollect in-depth information about a patients medical, functional, and social condition, to identify individual needs in order to develop a plan to meet those needs./liliWork with patient and family/caregivers to determine specific goals and actions based on assessment./liliCoordinate discharge planning. Execute specific interventions to meet established goals./liliOrganize, integrate and coordinate the necessary resources to accomplish the goals and plan./liliAssess the patients prior level of functioning, access to and/or use of community resources and available support systems./liliAssist the care team in developing a plan of care which includes, but is not limited to: assuring appropriateness of services and care setting, assuring individualized support and education, determining the need for continued services, planning for discharge, and identifying and connecting patients/families with available community resources if needed./liliCollaborate with Physicians and other members of the health care team on the patients behalf./liliIdentify appropriate admission and continued stay issues./liliEnhance the quality of patient care through effective and efficient use of resources./liliHelp to identify strategies for reducing length of stay and appropriate utilization of services./liliUtilize criteria including clinical pathway data and implement strategies to resolve controllable variances./liliAttend, facilitate and participate in rounds and case conferences./liliAdvocate for patient rights./liliMonitor efficiency and availability of services and evaluate outcomes through variance tracking, data collection./liliIdentify needs, facilitate, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles./liliParticipate in multidisciplinary groups and development of guidelines./li/ulpMinimum Qualifications:/pulliGraduate of an accredited school of nursing/liliBachelors Degree in Nursing (BSN)/liliCurrent RN license in the state of practice or compact license with the ability to obtain RN license in the state of practice/liliMinimum of three (3) years of experience as a Registered Nurse in an acute care setting/li/ulpPreferred:/pulliAt least five (5) years of nursing experience- Prior knowledge of managed care / utilization review/li/ulpOngoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies./ppFrequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations./ppManual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc./ppExpected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items./ppNeed to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc)./ppMay be expected to stand in a stationary position for an extended period of time./ppLocation: Holy Rosary Hospital/ppWork City: Miles City/ppWork State: Montana/ppScheduled Weekly Hours: 0/ppThe hourly range for this position is listed below.
