- Vermont Physical Therapist (PT) License Required Upon Acceptance
- Minimum 2 Years of Physical Therapy Experience Required
- Home Health Physical Therapy Experience with Patient Assessment, Treatment Planning, and Care Coordination Required
- Experience Completing OASIS Assessments and Home Health Clinical Documentation Required
- 37 Guaranteed Hours Per Week | Monday-Friday Day Schedule
- Provide Skilled Physical Therapy, Patient/Caregiver Education, Functional Assessments, and Individualized Plans of Care
- Coordinate Care With Physicians, Nurses, Rehabilitation Staff, Caregivers, and Other Clinical Team Members
- McKesson EMR Used; No Holiday Coverage Required
- Vermont PT License - Required Upon Acceptance
