Social Worker Responsibilities & DutiesConduct psychosocial assessments for new residentsDevelop individualized care plans in collaboration with healthcare teamsProvide individual and group counseling to residentsAssist residents in adjusting to nursing home lifeAdvocate for residents' rights and needsFacilitate communication between residents, families, and staffCoordinate discharge planning and follow-up servicesAssist families with understanding and navigating the healthcare systemOrganize and implement recreational and therapeutic activitiesMaintain accurate documentation and case recordsParticipate in interdisciplinary team meetingsOffer crisis intervention services as neededProvide support and resources for end-of-life careEnsure compliance with federal and state regulationsConduct family meetings and provide mediation when necessaryCoordinate with community resources for additional resident supportEducate staff on psychosocial issues affecting residentsMonitor and evaluate the effectiveness of care plansDevelop and implement policies to improve resident careSocial Worker Qualifications & SkillsMaster’s degree in social work (MSW)Licensed Clinical Social Worker (LCSW) credentialsExperience in geriatric or long-term care settingsKnowledge of Medicare and Medicaid regulationsStrong communication and interpersonal skillsExperience with case management and care coordination #J-18808-Ljbffr SPJST Senior Living
