In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence. Responsibilities Evaluate residents and develop individualized plans of care. Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions. Reassess patient progress and modify treatment plans as appropriate. Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations. Complete timely and accurate documentation using the electronic medical record (EMR). Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques. Collaborate with interdisciplinary team members to provide coordinated, patient-centered care. Maintain a productive clinical caseload while delivering exceptional therapy services. Qualifications Current State Physical Therapist (PT) License Current CPR/BLS Certification Valid driver's license, reliable transportation, and auto insurance Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required About Total Care Therapy Total Care Therapy is a therapist-owned and operated rehabilitation company providing Physical, Occupational, and Speech Therapy services throughout assisted and independent living communities.
