Leading MLTC in the NY area is seeking experienced REMOTE Care Managers CANDIDATES MUST RESIDE IN THE METROPOLITAN NEW YORK ARE A Responsibiliti es Perform assessments of members’ medical, psychosocial, physical and spiritual nee ds.Develops, implements, and monitors person centered service pl an.Ensures continuity of care across all healthcare settings by collaborating with the interdisciplinary care team, PCP, member/member’s representatives, and specialis ts.Utilizes benefit structure and community-based resources to meet member’s nee ds.Educates members on disease processes, preventative health, and quality of life interventio ns.Completes initial telephonic assessments and reviews UAS -NY visit data as assigned by the Supervisor of Coordinated Care in accordance with departmental workfl ow.
