Medical Social Worker (MSW)

Location: Everett, WA
Job Type: Contract
Duration: 13 Weeks
Shift: Day | 5x8 Hours
Schedule: 40 Hours/Week | Every Other Weekend | 72 Guaranteed Hours/2 Weeks
Local Candidates: Not Accepted
Traveler Candidates: Accepted

Pay Rate:
Traveler: $45–$50/hr

Required Qualifications:

• Active Washington MSW license — Required

• Minimum 1 year of acute-care social work experience — Required

• Previous travel experience — Required

• Discharge planning and care coordination experience — Required

• BLS certification — Required

• Epic experience — Required

• AIDA EMR experience — Preferred

• Strong case-management experience in Med-Surg, LLOS, and ED — Preferred

• Strong psychosocial assessment, crisis intervention, and communication skills

• Ability to work effectively with patients, families, and interdisciplinary teams

Responsibilities:

• Conduct comprehensive psychosocial assessments for hospitalized patients

• Develop and coordinate discharge plans and transitions of care

• Collaborate with physicians, nurses, case managers, and community agencies

• Provide crisis intervention, counseling, and patient/family education

• Arrange referrals for rehabilitation, home health, hospice, and community resources

• Support patients and families with social, emotional, and discharge-related needs

• Maintain accurate and timely documentation in Epic

• Coordinate care with community and healthcare resources

• Participate in interdisciplinary care planning

• Float between hospital campuses and inpatient units as required

• Follow hospital policies, patient-safety standards, and social-work practices
