Description

• Conduct assessments for patients in both inpatient and outpatient environments, creating personalized treatment strategies
• Implement physical therapy interventions that focus on enhancing mobility, strength, balance, and overall function
• Collaborate with nurses, physicians, case managers, and fellow rehabilitation professionals to optimize patient outcomes
• Educate patients and their families about therapeutic goals, discharge processes, and home exercise routines
• Maintain meticulous documentation of evaluations, treatments, and patient progress, adhering to facility regulations and standards
• Engage in collaborative care meetings and interdisciplinary discussions to assure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent credential from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous, although not required
