Licensed Practical Nurse (LPN) OR Registered Nurse (RN)

Community Health Network, LTD (CHN) is looking for a Licensed Practical Nurse (LPN) OR Registered Nurse (RN) who wants to:

• Provide care to someone in their home
• Maintain a consistent caseload
• Work closely with an Administrative Team that appreciates them

Homecare Nursing requires strong communication but promotes growth and independence. No productivity levels or 45 bed assignments! Work for a growing, person-centered company:

Key Responsibilities:

• Person-centered care
• Strong documentation skills
• Electronic Health Record experience is a plus!
• Medication administration, updating orders and day-to-day care coordination.
• Effectively communicate with supervisors and guardians
• Dietary support and monitoring
• Relevant education and training based on the individuals care plan
• Complete all other duties as assigned

What we need you to have (not right away! But prior to our hiring you):

• Certificate in nursing from an accredited nursing school
• Licensed as a LPN/RN as issued by the State of Ohio-Registered on the State Nurse Registry
• BLS CPR
• High school diploma, High school transcripts, GED or equivalent
• Driver's License
• Two professional reference letters
• Auto insurance
• Social Security Card
