Job Title: LPN - Home Health

Location: Nashville, TN 37207

Duration: 3+ Months (possibilities of extension)

Shift: Monday thru Friday (8:00 AM to 4:30 PM)

Job Responsibilities:

Visiting nurse 6 to 7 patients a day
Using HCHB EMR
Medication reconciliation
Disease process teaching
Lab draws and wound care

Required Qualifications:

Associate's degree
3 years minimum home health with LPN license
HCHB experience

Additional Information:

Schedule: 8 hours per day / 40 hours per week
Productivity expectations: 30 points
On Call/Weekend Requirement: One weekend a month if needed with an RN also on call
Coverage Areas: Davidson, Wilson, Summer counties
Mileage reimbursed between visits: Yes
Nice to have: Home health experience with HCHB for a LHC company, flexibility, team player
Dress code: Solid color scrubs
