Registered Nurse - Case Manager

Job Purpose:- The Registered Nurse - Case Manager at Crescent Community Health Center is responsible for providing comprehensive case management services to patients, ensuring they receive high-quality, coordinated care. The role involves assessing patient needs, developing care plans, and facilitating access to healthcare resources, while promoting patient education and self-management.Key Responsibilities:- Conduct thorough assessments of patients' health status and needs to develop individualized care plans.- Collaborate with healthcare providers, patients, and families to ensure effective communication and coordination of care.- Monitor patient progress and modify care plans as necessary to achieve optimal health outcomes.- Educate patients and families on health conditions, treatment options, and self-management strategies.- Facilitate access to healthcare services and resources, including referrals to specialists and community support services.- Advocate for patients' rights and ensure they receive appropriate and timely care.- Maintain accurate and up-to-date documentation of patient interactions and care plans.- Participate in multidisciplinary team meetings to discuss patient care and improve service delivery.- Stay informed about current healthcare practices and regulations to ensure compliance and best practice standards.Required Education:- Bachelor of Science in Nursing (BSN) from an accredited institution.- Current and valid Registered Nurse (RN) license in the state of practice.Required Experience:- Minimum of 3 years of clinical nursing experience, preferably in a case management role.- Experience working in community health settings or with underserved populations is highly desirable.- Demonstrated experience in coordinating patient care and working collaboratively with healthcare teams.Required Skills and Abilities:- Strong assessment and critical thinking skills to effectively evaluate patient needs and develop appropriate care plans.- Excellent communication and interpersonal skills to interact with patients, families, and healthcare professionals.- Ability to manage multiple cases simultaneously and prioritize tasks efficiently.- Proficiency in using electronic health records (EHR) systems and other healthcare-related software.- Strong organizational skills and attention to detail to ensure accurate documentation and follow-up.- Ability to work independently and exercise sound judgment in decision-making.- Commitment to patient advocacy and providing high-quality, compassionate care. #J-18808-Ljbffr Crescent Community Health Center
