Physical Therapy Assistant (PTA)

Full-Time | Skilled Nursing / Long-Term Care | Topeka, Kansas

Are you a compassionate, resident-centered Physical Therapy Assistant (PTA) who enjoys helping older adults improve mobility, strength, balance, safety, and independence?

Above All Talent Solutions is conducting a search for a full-time PTA to join a collaborative rehabilitation team serving adult and geriatric residents in a skilled nursing, long-term care, and post-acute rehabilitation setting.

Position Overview

In this role, you will work under the direction of a licensed Physical Therapist to carry out delegated components of individualized physical therapy plans of care. You will help residents improve or maintain mobility, strength, endurance, balance, pain management, safety, and functional independence.

You will collaborate closely with Physical Therapy, Occupational Therapy, Speech-Language Pathology, nursing, providers, residents, families, and the broader interdisciplinary care team to support positive clinical outcomes and quality of life.

Responsibilities

• Implement delegated physical therapy interventions and components of established plans of care for adult and geriatric residents with orthopedic, neurologic, cardiopulmonary, balance, mobility, and functional limitations in a skilled nursing, long-term care, and post-acute rehabilitation setting, under the direction of a licensed Physical Therapist.
• Provide hands-on therapeutic interventions, including therapeutic exercise, gait training, transfer training, balance activities, functional mobility training, endurance activities, pain-management strategies, and assistive-device training.
• Monitor resident response to treatment, communicate observations, progress, changes in condition, barriers to participation, and clinical concerns promptly to the supervising Physical Therapist and interdisciplinary care team.
• Complete timely, accurate treatment documentation, daily notes, progress updates, and resident-response records in accordance with facility policy, payer requirements, and applicable regulatory standards.
• Reinforce education provided by the Physical Therapist regarding prescribed exercise, transfers, safe mobility, fall-prevention strategies, positioning, and use of walkers, canes, wheelchairs, braces, or other assistive devices.
• Participate in care conferences, interdisciplinary care planning, quality-improvement initiatives, and program-development activities to support strong clinical and operational outcomes.
• Provide occasional coverage at nearby facilities, if required by the role and confirmed by the employer. (Remove this bullet if the role is strictly single-site.)

Qualifications

• Graduate of a CAPTE-accredited Physical Therapist Assistant program.
• Current, unrestricted Kansas Physical Therapist Assistant certificate, or eligibility to obtain Kansas certification before the start date.
• New graduates are encouraged to apply; relevant clinical fieldwork or prior experience in skilled nursing, long-term care, post-acute rehabilitation, inpatient rehabilitation, or acute care is a plus.
• Strong communication, documentation, organization, clinical-observation, and interdisciplinary-collaboration skills.
• A compassionate, resident-centered, outcomes-focused approach to care.
