RN Field Clinician - Senior Community Care - Hartford, Tolland - CT Per Diem

Overview Optum Home & Community Care is creating an integrated care model to address physical, mental and social needs. As part of the Senior Community Care (SCC) team, the RN provides care to patients at home, in nursing homes and in assisted living settings, supporting care access and coordination. $2,500 Sign On Bonus For External Candidates Weekday M-F availability, within 8a-8p timeframe preferred Standard Hours: 24 hours minimum per month Primary Responsibilities Collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination Triage member needs to the appropriate care team member and level of care Assist with transitions in care: outpatient and inpatient Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments What Makes an Optum Career Different? Required Qualifications Associate degree or higher in Nursing (RN) Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire Certified in Basic Life Support or ability to obtain by start date Driver's license and access to reliable transportation that enables travel up to 100% to visit clients and/or patient sites within a designated area Preferred Qualification BSN degree Pay is based on several factors including location, education, work experience, and certifications. The hourly pay range for this role is $28.94 to $51.63 per hour for full-time employment.
