Are you an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a passion for accuracy, collaboration, and quality resident care? Rock Creek Rehabilitation & Healthcare Center is seeking a dedicated MDS Coordinator to join our clinical leadership team.
In this role, you'll play a vital part in ensuring accurate resident assessments, individualized care planning, regulatory compliance, and quality outcomes while working alongside a supportive interdisciplinary team.

Coordinate and complete the MDS assessment process in accordance with state and federal regulations
Develop and maintain individualized resident care plans with the interdisciplinary team
Ensure accurate and timely completion of all MDS documentation
Monitor assessment schedules and regulatory deadlines
Review clinical documentation to support quality outcomes and appropriate reimbursement
Collaborate with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care
Assist with survey readiness and regulatory compliance
Maintain compliance with facility policies and procedures
Perform other duties as assigned
Current Colorado RN or LPN license in good standing (or compact state license, if applicable)
CPR Certification
MDS experience preferred
Long-term care experience preferred
Strong organizational, communication, and documentation skills
Ability to work collaboratively with an interdisciplinary team
Passion for delivering exceptional resident-centered care
