MDS Coordinator (RN/LPN)

Are you an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a passion for accuracy, collaboration, and quality resident care? Rock Creek Rehabilitation & Healthcare Center is seeking a dedicated MDS Coordinator to join our clinical leadership team.

In this role, you'll play a vital part in ensuring accurate resident assessments, individualized care planning, regulatory compliance, and quality outcomes while working alongside a supportive interdisciplinary team.

• Coordinate and complete the MDS assessment process in accordance with state and federal regulations

• Develop and maintain individualized resident care plans with the interdisciplinary team

• Ensure accurate and timely completion of all MDS documentation

• Monitor assessment schedules and regulatory deadlines

• Review clinical documentation to support quality outcomes and appropriate reimbursement

• Collaborate with nursing, therapy, dietary, social services, and other departments to ensure comprehensive resident care

• Assist with survey readiness and regulatory compliance

• Maintain compliance with facility policies and procedures

• Perform other duties as assigned

• Current Colorado RN or LPN license in good standing (or compact state license, if applicable)

• CPR Certification

• MDS experience preferred

• Long-term care experience preferred

• Strong organizational, communication, and documentation skills

• Ability to work collaboratively with an interdisciplinary team

• Passion for delivering exceptional resident-centered care
