Heart Failure Care Navigator: Transitions & Readmissions

The Ohio State University’s Ross Heart Hospital is seeking a Transition Nurse Navigator within the Heart Failure Program to coordinate care across the continuum. You will collaborate with physicians, nurses and post-acute partners to optimize discharge planning and reduce readmissions through patient-centered education and timely follow-up. The ideal candidate is a registered nurse with BSN (MS preferred), at least five years of clinical experience, and proven expertise in care coordination, #J-18808-Ljbffr Ohio State University
