Physical Therapist (PT) – Full‑Time Elderwood at Burlington – Burlington, VT Overview We are seeking a full‑time Physical Therapist (PT) to join our dynamic rehabilitation team and help residents regain strength, mobility, and independence. This position is responsible for administering skilled physical therapy treatments and ensuring the highest degree of quality care at all times. A state‑specific physical therapy license is required. Elderwood at Burlington is a skilled nursing and rehabilitation facility offering short‑term rehab, post‑acute care, and long‑term care services. Located in the heart of Burlington, the facility is known for its strong clinical outcomes, collaborative interdisciplinary approach, and commitment to personalized care. Our rehab team plays a key role in helping residents safely transition from hospital to home while maximizing independence and quality of life. Responsibilities Evaluate and develop individualized treatment plans for residents. Deliver skilled physical therapy services in accordance with physician orders. Collaborate with interdisciplinary team members to optimize care plans. Monitor, document, and adjust treatments based on resident progress. Educate residents and families on therapy goals and outcomes. Initiate skilled physical therapy treatments in accordance with physician’s orders; maintain required contact with physician. Maintain written evaluations, progress notes, and discharge summaries for individuals in the physical therapy program. Confere with other health‑care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function. Provide input for meetings and attend during absences of the Director Physical Therapy. Maintain statistical records of individuals’ treatments. Assist with in‑service training programs upon request. Record and update individuals’ plan of care in the Inter‑disciplinary Kardex. Submit requisitions for supplies needed in treatment to the Director Physical Therapy. Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation. Qualifications Doctorate or master’s degree in Physical Therapy from an accredited program. Current Vermont Physical Therapy license (or eligibility). Strong clinical, communication, and documentation skills. Passion for working with geriatric and post‑acute populations. Graduation from an approved school of physical therapy required. State license to practice physical therapy required. Two years’ experience in physical therapy preferred. Ability to direct others if necessary. Good command of English at an intermediate or advanced level to interact safely and effectively with residents, coworkers, visitors, and supervisors.
