Job Details: Job Title: Home Health Physical Therapy Assistant Location: Denver , CO Setting: Home Health Pay Range: 1600-1700 Schedule: M-F 8a-5p Contract Length: 13 weeks Candidate Type: Traveler or local candidates accepted Requirements: 1 year of Medicare certified Home Health experience. 1 year of HCHB EMR experience. Responsibilities: Provide one-on-one physical therapy services to patients in their homes. Complete patient assessments and document progress toward therapy goals. Develop and adjust individualized care plans based on patient needs and response to treatment. Coordinate care with families and other healthcare team members to support patient outcomes.
