Job Summary The LPN Float position is a flexible, field-based role that supports both home care and home health clients. This role requires travel within and outside of Philadelphia to deliver high-quality nursing care while adhering to all clinical and documentation standards. Key Responsibilities Provide skilled nursing care to home care and home health clients, including wound care, medication administration, monitoring patient conditions, and patient education. Collaborate with the care team to ensure coordinated and effective patient care. Respond promptly to changes in patient conditions and communicate updates to the care team. Complete all patient visit documentation accurately and submit it within 24 hours of the visit. Maintain accurate, up-to-date records in compliance with agency policies and regulatory requirements. Accept all assigned visits and complete them in a timely manner. Adhere to all federal, state, and agency regulations and standards. Attend required training sessions and team meetings as needed. Qualifications Valid LPN license in the state of Pennsylvania. Minimum of 1 year of nursing experience (home care or home health preferred). Strong organizational and time-management skills.
