Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and craft personalized treatment plans
• Implement physical therapy interventions centered around mobility, strength development, balance, and overall functional improvement
• Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Provide education to patients and families regarding therapeutic objectives, discharge planning, and prescribed home exercises
• Accurately document evaluations, treatments, and patient progress in alignment with facility and regulatory standards
• Engage actively in care team meetings and interdisciplinary conferences to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• Preferred CPR certification
• Advanced certifications, such as neurological or geriatric, are advantageous but not required

Skills

• Preferably, prior experience in a similar role
