We re seeking Nurse Practitioners or Physician Assistants (Advanced Practice Provider) who thrive in high-touch, patient-centered care and enjoys making a lasting impact on the lives of elderly patients.

About the Role

As an APP on our team, you ll serve as the clinical lead for your patients delivering in-home primary, urgent, and palliative care. You ll work closely with physicians, social workers, paramedics, and care coordinators to ensure each patient receives thoughtful, personalized care. This is a full-time role with a schedule of Monday through Friday, no weekend work required.

Key Responsibilities

• Deliver primary and palliative care to elderly patients in their homes (3 6 patients per day).
• Perform physical assessments, develop care plans, and manage chronic and acute conditions.
• Provide education and counseling to patients and families.
• Manage medications, labs, imaging orders, and referrals as needed.
• Participate in interdisciplinary team meetings and care plan reviews.
• Maintain meticulous documentation to meet care quality and compliance standards.

Experience

• 3+ years of progressive NP or APP clinical experience
• Strong background in geriatrics, internal medicine, palliative care, or end-of-life care
• Home-based care experience preferred but not required
