Life Care Centers

Life Care Centers seeks an MDS Coordinator to support our Healthcare & Medical Services team. In this role, you will manage and oversee the Minimum Data Set (MDS) assessment process, ensuring accurate, timely documentation and regulatory compliance. You will collaborate with nursing, therapy, and interdisciplinary teams to develop and update care plans that prioritize resident needs and outcomes. This position offers a patient-first, family-focused culture with opportunities for professional growth, teamwork, and meaningful impact on the quality of care for our senior residents.
