Pay Range: $37.45 - $44.95 per hour based on experience Schedule: Friday 8am - 5pm and participation in on‑call rotation (evenings and weekends); occasional weekends. Current Colorado Registered Nurses (R.N.) License, Cardiopulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred. Must have a valid driver’s license, proof of insurance and have means of transportation. What You’ll Be Responsible For In coordination with the Care Manager and other nursing staff, develop and implement a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintain aligned, comprehensive plan(s) for all participants and coordinate care between facility and the Care/Interdisciplinary Team. Conduct a comprehensive initial nursing assessment with new PACE enrollees. Coordinate with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conduct nursing re‑assessment semi‑annually and as needed. Perform general and skilled nursing care to participants as assigned. Implement the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observe, record, and report participant’s condition and reaction to drugs and treatments to providers. Triage participants to determine priority of care and assist in the treatment, examination, and testing of participants. Participate in nursing on‑call rotation to provide after‑hours and weekend assistance and care, as needed, to PACE participants.
