The VNA Health Group seeks a compassionate Full Time Registered Nurse Hospice Liaison to join our dedicated Hospice Team in Middlesex County, NJ. You will assess patients for hospice eligibility, coordinate transitions to hospice care, and conduct admissions, serving as an advocate for patients, families, and referring partners. You’ll collaborate with an interdisciplinary team, support discharge planning at Community Medical Center, and promote access to hospice services through community #J-18808-Ljbffr VNA Health Group
