Schedule: Monday-Friday, 8:30 AM - 4:30 PM
Pay Rate: $24.00/hour

Job Description:

• Conduct comprehensive client assessments and identify barriers to stability and success.
• Develop, implement, and monitor individualized service plans.
• Connect clients to housing, employment, healthcare, behavioral health, financial assistance, and other supportive services.
• Provide advocacy, crisis intervention, and ongoing case management support.
• Maintain accurate and timely case notes, documentation, and program records.
• Collaborate with community agencies, government organizations, and service providers to coordinate services.
• Monitor client progress and adjust service plans as needed to ensure successful outcomes.
• Ensure compliance with program requirements, agency policies, and funding guidelines.

Job Requirements:

• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred.
• Minimum of 2 years of experience in case management, social services, housing services, or a related field preferred.
• Experience working with veterans, individuals experiencing homelessness, or underserved populations is highly desirable.
• Strong knowledge of community resources and social service systems.
• Excellent communication, relationship-building, and problem-solving skills.
• Ability to manage multiple cases while maintaining detailed and accurate records.
• Proficiency with Microsoft Office and electronic case management systems.
• Ability to work independently and collaboratively within a team environment.
• Valid driver's license and reliable transportation preferred.
