Job Description
Title: Physical Therapist - Rehabilitation

Schedule: Days (0700 - 1700)

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

Requirements:
- Current unencumbered Oregon Physical Therapy license required.
- Healthcare Provider Level BLS required.
- Must be able to travel to multiple points of care.

Roles & Responsibilities:
- Evaluate and treat patients with injuries or conditions affecting physical mobility or functional independence.
- Provide rehabilitation services in acute and swing bed settings as well as outpatient services including sports and orthopedic rehab, pediatric rehab, neurological and stroke rehab, balance and dizziness rehab, wheelchair seating, and neck, back, and extremity care.
- Develop individual treatment plans based on patient assessments and goals.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Maintain accurate documentation and records of patient progress.
