Home Health Physical Therapist Full Time

Position Overview Elevate patient care by providing skilled home health physical therapy services to residents in Lake County, Tavares, and Eustis. This full‑time role requires Monday‑Friday visits and focuses on helping patients recover from illness or injury in the comfort of their own homes. Essential Job Functions Perform initial and ongoing assessments of mobility, coordination, strength, and functional level. Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes. Provide written evaluations and comprehensive plans of care that define physical‑therapy needs, goals, progress, and expected outcomes. Deliver physical‑therapy services in accordance with prescribed physician orders. Maintain and update the plan of care by documenting client performance, adjusting goals and treatment as needed, and determining discharge care plans. Plan discharge, including collaborating with and educating patients and caregivers on home exercise plans. Coordinate services with the health‑care team—physicians, parent/guardians, nurses, and others—as required. Document patient status and treatment outcomes per policy and procedure, ensuring minimal supervision or instruction. Maintain accurate documentation that’s free of technical or clinical denials after payer review. Communicate effectively with staff, the health‑care team, and families. Serve as a clinical resource and consultant for staff. Supervise other personnel (PTA, HHA, OTA) as applicable. Complete OASIS assessments where permissible by state practice and regulation. Follow all policies, procedures, licensure regulations, and state laws governing patient care. Current, unrestricted state license as a Physical Therapist in the state of practice. Valid CPR and driver’s license. Preferred Experience in home‑health therapy. Completion of OASIS documentation.
