Position Summary Pine Acres is seeking a licensed Physical Therapist (PT) to join our rehabilitation team on a per diem or full-time basis . The ideal candidate is compassionate, motivated, and experienced in working within a skilled nursing or rehabilitation setting. Responsibilities Evaluate and develop individualized treatment plans for residents Implement physical therapy interventions to improve mobility, strength, balance, and functional independence Collaborate with physicians, nursing staff, and therapy team members Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory guidelines Educate patients and families on therapy plans and home exercise programs Ensure compliance with state, federal, and facility policies Qualifications Current Iowa license as a Physical Therapist Graduate of an accredited Physical Therapy program Experience in skilled nursing or rehab setting preferred Strong clinical, documentation, and communication skills Ability to work independently and as part of a team Benefits (Full-Time) Competitive salary Health, dental, and vision insurance Paid time off 401(k) Continuing education opportunities Supportive team environment Per diem employees enjoy flexible scheduling options.
