Pay: $45.00 per visit Job Type: W-2 Per diem Work Location: On the road Medical Specialty: Home Health / Hospice We are looking for a clinically experienced nurse who has nursing documentation and charting skills, is a fast learner, and has strong communication skills. This opportunity is open for a highly motivated and positive-minded individual. The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments and a variety of medically related tasks. The LVN is a licensed nurse who works in a Home Care setting to provide nursing services. Availability: Our patients need care 7 days a week. Flexibility is allowed, but you must be able to work at least two‑three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Responsibilities Providing nurse visits to home health patients. Care for ill, injured, or convalescing patients by implementing the plan of care initiated by RN. Provide basic patient care and treatment. Document consistently with the plan of care. Communicate with patients to schedule visits. Educate the patient on how to take care during non-visit days. Demonstrate knowledge of regulatory requirements. Experience Visiting LVN for Home Health: 1 year (Required) Nursing: 1 year (Required) Current CA LVN License (Required) BLS from the American Heart Association and/or the American Red Cross (Required) NSO Nursing License Insurance (Required)IV/Phlebotomy Certified (Preferred) Work Location: The city close to you, with a cluster of patients who need care for that day This position is currently accepting applications. #J-18808-Ljbffr
