Care Plan Manager

Role Description The Care Plan Manager is a full-time, on-site role based in New Braunfels, TX, responsible for coordinating and overseeing individualized care plans for clients or patients. In this role, the Care Plan Manager evaluates needs, develops comprehensive care strategies, and ensures that services are delivered consistently and effectively. Day-to-day responsibilities include conducting assessments, documenting care objectives, monitoring progress, and adjusting plans as needed concerns, and maintain high-quality standards of care. This position also involves maintaining accurate records, adhering to regulatory and organizational guidelines, and contributing to process improvement initiatives related to care management. Qualifications Demonstrated experience in care coordination, case management, or care planning within healthcare, social services, or a related field. Strong communication, interpersonal, and relationship-building skills to work effectively with clients, families, and professional teams. Proficiency in documentation, organization, and time management to handle multiple care plans and priorities accurately. Ability to analyze information, make sound decisions, and adapt care strategies based on changing client needs and clinical guidance. Comfort with technology, including electronic records systems, spreadsheets, and standard office software. Knowledge of relevant regulations, care standards, or best practices in patient or client care is highly beneficial.
