We are now hiring for the Patient Coordinator role in our Westminster, CO location! The Patient Coordinator is vital in ensuring effective communication and support between our healthcare team and patients. Responsibilities The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources. Accompany patients to their benefit renewal appointments to advocate for the patient and the program. Visit patient homes and update patient care. Effectively communicate and provide information to patients, caregivers, and physicians. Understand HIPAA and maintain confidentiality. Display a warm and friendly attitude. Minimum Qualifications 1 to 3 years of experience in a Care Team or Patient Navigation Role preferred Healthcare experience preferred Key competencies include, empathy, problem solving, communication, active listing, and tenacity Self-motivated with the ability to manage time appropriately. Experience with MS Office products (Outlook, Word, Excel, etc.) Minimum associate level education preferred. (High school diploma or equivalent is required.) 1 year of medical knowledge preferred.
