All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope.

Position: Field Physical Therapist

Key Responsibilities:

• Provides comprehensive, skilled physical therapy services/interventions in accordance with physician orders.
• Comfortable with start of care (SOC), Evaluations, Resumption of Care (ROC), Recertification (Recert), and Discharges (DC).
• Assesses patient skilled needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with the interdisciplinary team to plan, evaluate team's goals for each individual patient.
• Educates, instructs, and trains patient, family, responsible caregiver on patient's physical therapy program.

Qualifications:

• Active/Registered by the Physical Therapy Board of California.
• Graduate from a Physical Therapy accredited school.
• Strong documentation skills.
• Excellent communication and interpersonal skills.
• Willingness to travel to patient locations for evaluations.
• Flexibility regarding work locations.

REQUIREMENTS:

• California clinical license.
• CPR certification.
• CA driver's license.
• CA car insurance.
• Required Physical, TB test, Vaccines/Titers.
• Two professional references.
