Hospice RN Case Manager (Manhattan)

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospice receives patients from referring institutions throughout New York City and the entire tristate area. Qualifications Current NYS Registered Nurse license. Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice. At least one (1) year of community hospice or home health experience is preferred. Hospice experience preferred. Position requires ability to commute to multiple territories to ensure continuity of care. A valid driver’s license is required. (This requirement does not apply to positions assigned exclusively to Manhattan territory) A vehicle is required to commute to patients.
