Physical Therapist

This role provides general Physical Therapy evaluations and treatments that impact patients' functional abilities. The Physical Therapist oversees and directs the work of Physical Therapist Assistants and/or other designated staff members. The therapist provides general physical therapy evaluations and treatments for a wide range of ages with various diagnoses and issues. Assessment is performed to identify therapy diagnosis, formulate a treatment plan with goals, and implement physical therapy services. The therapist implements, supervises, and modifies the therapy plan of care based on patient response to optimize outcomes. Clinical records and case histories are reviewed regularly. Attendance at care conferences, regular meetings, and rounds is required to remain in contact with referring providers and other healthcare professionals. The Physical Therapist collaborates with team members to ensure appropriate medical equipment is arranged and care plans are established prior to discharge. Documentation of patient encounter findings and treatments, including diagnoses, treatment plans, recommendations, and prognoses, is necessary per organizational policies. Services are to be provided in a timely manner and according to regulatory requirements. Work involves both in-patient and out-patient populations in various settings as assigned. The Physical Therapist directs the work of support staff, students, or interns. Facilitation of a healthcare partnership between providers and patients and their families is key. Developing, implementing, and conducting patient education and therapy programs based on patient needs is essential. Patients and families should receive clear and concise home exercise programs to achieve rehabilitative potential. Feedback should be obtained from patients or caregivers to ensure understanding of instructions. Advising patients and family members on methods that facilitate self-care and independence fosters a partnership between providers and patients. Home programs created should be clear and concise, and understanding should be confirmed through feedback and demonstration. Assistance to senior staff in administrative matters and clinic operations is part of the role. Utilization of the hospital computer system to obtain patient information and stay updated on directives is expected. Participation in the timely and efficient flow of information through digital platforms is required. Summaries of results, statistics, and metrics may be necessary as required by management. Assisting in the acquisition and fitting of durable medical equipment/devices based on patient needs is part of the role. Communication with supervisors and/or equipment providers to measure, fit, and deliver equipment is essential. Maintaining supply stocks and clinical equipment at acceptable levels is necessary. Current knowledge of billing procedures and mechanisms is important to aid in reimbursement for services. Maintaining cleanliness of clinical space is a requirement. Educating staff on PT-related issues and educating patients/families about functional limitations is key to achieving desired goals. Participation in committee and management duties as requested is expected. Therapy consultation, presentations, and in-services to medical staff and other interdisciplinary healthcare personnel may be required. Adherence to regulatory and practice standards of PT set by professional associations is necessary. Following confidentiality and quality assurance standards is essential in all aspects of work. Other duties as assigned may be required.
